• This meta-analysis found no overall relationship between cultural identity and substance use among Indigenous youth. • Results challenge assumptions that cultural identity is uniformly protective against substance use. • Findings underscore the importance of examining specific dimensions of cultural identity and contextual factors. • Clarifying when and how cultural identity supports resilience can inform culturally grounded prevention efforts.
