• A post-clearance nomogram was developed for patients with multiple penile condyloma acuminatum after 5-ALA photodynamic therapy. • The model predicted 12-month clinical recurrence using virological, lesion-related, host-related, and treatment-course variables. • Multiple HPV infection, high-risk HPV positivity, wart number, mucosal involvement, immunosuppression, and previous treatment history were associated with higher recurrence risk. • The model showed good internal discrimination, with an apparent AUC of 0.87 and an optimism-corrected AUC of 0.85. • This nomogram should be used for post-treatment follow-up planning, not as a pretreatment decision rule or evidence for additional PDT sessions.
