• A new stimuli-responsive hydrogel is developed to target inflammatory macrophages. • Release of MyD88 inhibitor is controlled by an MMP-12 linker. • The released inhibitor product is taken up by macrophages and retains its bioactivity. • The therapeutic hydrogel inhibits the foreign body response in a mouse model.
