• This review synthesizes current knowledge on Aril Paleness Disorder (APD) in pomegranate, emphasizing its complex genetic, physiological, environmental, and nutritional determinants. • The phenylpropanoid pathway appears to be the primary target affected by this disorder. • Molecular evidence highlights disrupted anthocyanin biosynthesis, particularly reduced UFGT and OXKF genes expression, as the most probable mechanism underlying APD. • Heat stress acts as a key environmental trigger, leading to nutritional imbalances and ultimately compromising the integrity of structural and regulatory proteins. • An integrated management framework is proposed, combining tolerant cultivar selection, targeted foliar nutrition, canopy microclimate regulation, and optimized irrigation practices.
