• Myopic eyes with ocular hypertension (OHT) exhibit significantly stiffer corneal biomechanics compared to normotensive myopic controls, prior to any detectable glaucomatous damage. • This increased corneal stiffness is independently associated with a tendency toward reduced retinal microvascular perfusion in both the optic disc and macular regions. • The early link between tissue stiffening and microcirculatory impairment highlights the potential value of combining biomechanical and OCTA assessments for risk monitoring in myopic OHT.
