Association of B12, folate, and iron parameters with microcytosis and macrocytosis in individuals over 65 years of age registered to a home care unit

Anemia significantly impairs the quality of life and overall health status of the geriatric population. This study aims to investigate microcytic and macrocytic disorders along with anemia risk factors in home care patients aged 65 and over to facilitate early diagnosis and treatment. This study was conducted through a retrospective review of the records of 614 individuals aged over 65 registered in the Home Care Unit of a tertiary hospital in Türkiye. For participants meeting the inclusion criteria, complete blood count, vitamin B12, folate, iron, iron-binding capacity, and ferritin levels from blood tests performed between January 1, 2023, and December 31, 2023, were analyzed alongside age and gender data. Half of the 614 participants were in the late-elderly period. In the total population, the prevalence of microcytic and macrocytic was 7.7% and 13%, respectively. The prevalence of anemia was found to be 57%; both the prevalence and severity of anemia increased significantly with age. Among non-anemic individuals, cobalamin, folate, and ferritin levels were within normal ranges at rates of 45%, 44%, and 46.7%, respectively. Iron deficiency was found to be closely associated, particularly with moderate and severe anemia. In complementary regression analyses, iron deficiency was associated with microcytosis and higher anemia severity. The prevalence of anemia in this study was found to be remarkably high; furthermore, deficiency rates of associated parameters were also high even in non-anemic individuals. Therefore, we concluded that prospective and etiology-oriented screening studies are necessary to prevent anemia and its associated complications in the elderly.
