• Elevated sd-LDL-C is independently associated with the presence of OCT-defined PR in the culprit lesion of patients with acute coronary syndrome. • Higher quartiles of sd-LDL-C are persistently associated with PR even after multivariable adjustment, independent of LDL-C levels. • The co-existence of high sd-LDL-C and high hs-CRP is associated with a higher frequency of PR in the culprit lesion, representing the additive impact of lipid quality and systemic inflammation on plaque morphology. • OCT-defined morphological features of plaque vulnerability, such as thin-cap fibroatheroma, are more prevalent in the culprit lesions of patients with elevated sd-LDL-C and hs-CRP.
