Associations of triglyceride glucose index and atherogenic index of plasma with plaque, inflammation, flow, and functional ischemia assessed by cardiovascular computed tomography imaging in hypertensive patients: a study of different glucose metabolism status

The triglyceride glucose (TyG) index and atherogenic index of plasma (AIP) are indicators of insulin resistanceand atherosclerosis, respectively. Nevertheless, their relationship with coronary plaque, inflammation, flow and functional ischemia in hypertensive individuals with different glucose metabolism status has not been studied. The study involving 1752 hypertensive participants underwent coronary CT angiography (CCTA), stratified by diabetes mellitus (DM) or the biomarker quantiles. Plaque volume (lipid, fiber–lipid, fiber, calcification), burden, plaque characteristics, CT-derived fractional flow reserve (FFRCT), and fat attenuation index (FAI) were compared across groups. Spearman and logistic regression analysis to assess the association of biomarkers with CCTA parameters. Baseline analysis revealed that the group with the greatest TyG index and AIP had lower FFRCT, higher plaque volume (PV), and FAI than the lowest group (p< 0.001). Spearman analysis revealed that TyG index and AIP were positively correlated with the PV and FAI, and negatively correlated with FFRCT (p< 0.05). In DM participants, the correlation between TyG index and calcified PV and FAI of LAD and RCA was weaker, as was the correlation between AIP and calcified PV (p> 0.05). AIP and TyG index proved independently linked to functional ischemia, regardless of DM (p< 0.001). Among hypertensive participants, significant correlations were observed between the AIP, TyG index and coronary plaque, flow, and inflammation. The coexistence of DM has different effects on these associations. Regardless of DM, functional ischemia was independently associated with both TyG index and AIP.
