Automatic collateral quantification in acute ischemic stroke using U2-net

目标:利用U-2-Net深度学习框架,通过计算机断层扫描血管成像(CTA)图像对急性缺血性卒中(AIS)的侧支循环进行自动化定量,并比较其与传统视觉侧支评分(vCS)的性能。方法:收集118例确诊AIS病例,分为94例开发集和24例测试集。CTA图像经过预处理和标注。U-2-Net被训练用于分割侧支血管,根据受影响半球与健康半球的血管体积比生成定量侧支评分(qCS)。通过Dice相似系数(DSC)、Spearman相关系数、组内相关系数(ICC)和准确率评估性能,并与vCS(Tan和Menon评分)及金标准进行比较。结果:U-2-Net展现出稳健的分割能力,测试集平均DSC为0.75。qCS与vCS呈强相关,rho值范围为0.78至0.92。与更精细的六分类Menon评分相比,qCS表现出更强的一致性(开发集:ICC = 0.83,测试集:ICC = 0.93),优于与四分类Tan评分的比较(开发集:ICC = 0.76,测试集:ICC = 0.79)。在分类准确率方面,AI模型在四分类中与金标准和vCS的准确率分别为0.83和0.71;在二分类中,准确率提升至0.88和0.83,强调其区分侧支状态的能力。结论:我们的U-2-Net AI模型为AIS侧支循环的定量提供了一种可靠、客观的工具。qCS与vCS高度一致,证实了自动化侧支评估的可行性,可能提高诊断准确性并优化治疗决策。
