• Glioma neoantigen immunotherapy is fundamentally constrained by HLA-Ia loss and impaired cellular immune surveillance. • HLA-E, a non-classical HLA-Ib molecule functions as an alternative antigen presentation platform that reshapes NK and CD8+ T-cell responses through the NKG2A axis. • γδ T cells provide an HLA-independent effector arm that complements neoantigen-driven adaptive immunity under immune escape conditions. • AI-assisted neoantigen engineering and BBB-penetrating delivery technologies enable translational realization of non-classical HLA- and γδ T-cell–based strategies. • A multidimensional cellular immune framework is proposed to overcome immune evasion in gliomas.
