• This study compares the impact of intraoperative ephedrine versus phenylephrine on postoperative acute kidney injury (AKI) after elective noncardiac surgery. • Intraoperative phenylephrine was associated with a higher risk of developing postoperative AKI than ephedrine. • This suggests that intraoperative vasopressor choice may be a modifiable factor in improving postoperative renal outcome.
