• Cortical ADC profiling revealed subtle abnormalities in IIH. • Baseline differences were focal rather than globally distributed. • Raw profiles outperformed CSF-adjusted profiles in this setting. • VP shunting was followed by increased superficial tissue-side ADC. • The clearest follow-up difference was seen in the 1.5–3.0 mm range.
