A 69-year-old woman treated for biliary sepsis developed a rapidly progressive pustular eruption with systemic involvement while receiving antibiotics. The case illustrates the diagnostic challenge of distinguishing between acute generalized exanthematous pustulosis and other severe cutaneous adverse reactions, requiring integration of clinical, laboratory and histological findings. It highlights the importance of early recognition, drug withdrawal and supportive management in achieving recovery.
