• Sluggish-flow-related artifacts mimic atherosclerotic plaque on HR-VWI, confounding stroke risk assessment. • Central enhancement pattern achieves 100% sensitivity for identifying sluggish-flow-related artifacts. • Posterior location and quantitative signal metrics independently discriminate artifacts from true plaque. • A stepwise algorithm integrating HR-VWI and luminal imaging improves discrimination accuracy.
