• Serum oxidative stress was elevated in patients with pemphigus and decreased after one month of therapy. • Circulating oxidative stress levels correlated with clinical disease severity. • 4-HNE, a lipid peroxidation marker, was strongly expressed in epidermal keratinocytes of lesional skin, especially in acantholytic areas. • Oxidative stress acts as a potential driver of keratinocyte damage and the pathogenesis of pemphigus.
