• DHRS not only participates in the metabolism of lipids, steroids and retinol, but also plays a significant role in tumorigenesis and progression. • We conducted a comprehensive systematic review of DHRS members across diverse malignancies, elucidating their associations with clinical drug resistance and chemotherapy response biomarkers. • The role of DHRS has been elaborated from the aspects of metabolic reprogramming, classical signal pathways and clinical potentials.
