• Gynecologic surgical site infection rates increased before multidisciplinary intervention. • Enterprise risk management guided risk assessment and staged infection prevention. • Surgical site infection rates decreased from 1.39% to 0.86% during implementation. • Infrastructure, process, and feedback interventions were implemented cumulatively. • Findings were associated with implementation but did not establish causality.
