• Five-year overall survival was similar with HI-IS/HSCT and standard care. • HI-IS/HSCT improved 5-year EFS versus modern standard care (76% vs 46%). • HI-IS/HSCT improved 5-year PFS versus standard care (82% vs 40%). • HI-IS/HSCT stabilized lung function and improved skin fibrosis at 24 months. • Older age and cardiac involvement predicted poorer outcomes after HI-IS/HSCT.
