• Heart rate variability (HRV) during tilt test was used to differentiate disorders of consciousness (DOC) from emergence. • Low frequency, high frequency, and standard deviation of normal-to-normal intervals were potential discriminative metrics. • HRV during tilt test exhibited significant differentiation potential in DOC.
