• IgA-based clustering revealed a high-risk MASLD subgroup with increased liver-related events. • Elevated serum IgA independently predicted liver-related events beyond fibrosis stage. • Multiple lines of evidence suggested enhanced IgA-associated immune responses in both the gut and liver. • IgA-expressing B-lineage cells were enriched in portal regions of MASLD livers. • Spatial transcriptomics revealed IGHA1 co-expression with immune- and extracellular matrix–related genes.
