• Large cohort of late CMV reactivation and disease after LTV prophylaxis in CBT. • Late CMV reactivation was frequent, with a 1-year cumulative incidence of 47.9%. • Acute GVHD and delayed hematopoietic recovery increase late CMV reactivation/disease. • These risk factors also increase the clinical burden of CMV management. • Extended LTV prophylaxis is recommended for CBT and essential for high-risk cases.
