• Multimodal IPC bundles reduced MDR Gram-negative acquisition in ICUs. • No individual IPC component showed a significant independent effect. • Component network meta-analysis identified promising IPC combinations. • Evidence certainty was low because of heterogeneity and indirect evidence. • Comparative trials are needed to optimize ICU infection prevention.
