• Inhaled protein nanoparticles (HbRT) are engineered for synergistic IPF therapy. • HbRT exerts a dual-targeting effect on the ROS/TGF-β1/SMAD signaling pathway. • HbRT alleviates oxidative stress, inflammation, EMT, and fibroblast activation. • Inhaled HbRT demonstrates potent efficacy in a bleomycin-induced IPF mouse model. • Inhaled HbRT demonstrates potent efficacy in human pulmonary fibrosis organoids.
