• Accelerometer-derived sleep duration, MVPA, and LPA were non-linearly associated with CVD incidence and all-cause mortality. • Suboptimal sleep duration and low PA were synergistically associated with an increased risk of CVD and all-cause mortality. • Increasing both MVPA and LPA may help to decrease the risks of CVD and mortality induced by suboptimal sleep duration.
