• LGALS1 is significantly upregulated in ccRCC, correlates with advanced stage, metastasis, and poor prognosis, and serves as an independent prognostic factor. • LGALS1 enhances ccRCC cell proliferation, migration, invasion, and lipid accumulation, driving sunitinib resistance in vitro. • LGALS1 activates the p38/ERK pathway to suppress PPARα/CPT1A, inhibiting fatty acid oxidation and promoting lipid metabolic reprogramming. • Inhibiting LGALS1 with OTX008 reverses sunitinib resistance, suppresses Subcutaneous tumor growth and lung metastasis in vivo.
