• Trophoblastic ferroptosis, driven by iron overload and lipid peroxidation, plays a key role in recurrent miscarriage pathogenesis, impairing placental development and immune tolerance. • Biomarkers such as elevated ferritin/MDA and reduced GPX4, alongside omics signatures, enable early RM detection and risk stratification. • Therapeutic strategies including iron chelators (e.g., deferoxamine) and ferroptosis inhibitors (e.g., ferrostatin-1) show promise in preclinical models for improving pregnancy outcomes. • Future directions emphasize trophoblast-specific inhibitors, multi-omics integration, and personalized therapies based on maternal iron phenotypes to mitigate RM.
