    • The incidence of frailty among elderly colorectal cancer patients is high, with nearly half of the patients already experiencing frailty. • Social isolation directly predicts frailty through the mediating effects of depression and health-promoting lifestyles, and indirectly predicts frailty through the chain mediation of depression and health-promoting lifestyles. • This study emphasizes the importance of addressing social isolation, as well as improving depression management and health-promoting lifestyles in elderly colorectal cancer patients, which may help prevent frailty.
