Breast cancer is a leading malignant tumor threatening women’s health. The co-occurrence of breast cancer and multiple myeloma is clinically rare. We herein report a 56-year-old female patient with left breast invasive ductal carcinoma who developed secondary IgA-λ multiple myeloma one year after breast cancer treatment, presenting with fever and pancytopenia. She received targeted myeloma treatment and remains under continuous therapy. This case reminds clinicians to screen for secondary hematological malignancies in breast cancer patients with unexplained atypical symptoms during follow-up.
