Bispecific antibodies are newly available medicines for multiple myeloma. They help the immune system recognize and attack myeloma cells, but their use in daily practice can be challenging, especially in patients with complex clinical conditions. This study collected the views of Italian myeloma experts on how to use bispecific antibodies targeting B-cell maturation antigen in patients whose disease had relapsed or become refractory after using the main available treatment classes. Using a structured consensus process, experts agreed on several practical aspects, including outpatient treatment under specific circumstances, reducing dosing in responding patients, and use in frail patients or in those with high-risk disease or organ involvement. These recommendations may support more consistent and safer clinical decisions on the use of bispecific antibodies targeting B-cell maturation antigen and help identify questions for future research.
