• Biased behaviours reduced trust, understanding, and perceived physician competence. • Prevalence: 3.7% patients and 2.1% physicians reported biased behaviour. • Language barriers and LGBTQ identity are risk factor for patient. • Ethnic minority status was not associated with higher risk. • Women physicians were more often exposed to biased behaviours.
