• The EDB antibody was an engineered anti-EDB-FN antibody fragment, for theranostic management of radioactive‑iodine-resistant thyroid cancer (RAIR-TC). • 89Zr-DFO-EDB provided high-contrast PET imaging independent of NIS expression, with tumor uptake peaking at 13.8%ID/g. • Single 100 μCi dose of 177Lu-DTPA-EDB induced 78.5% tumor regression and 65% necrosis in CAL-62 xenografts with only transient 5% weight loss. • Favorable labeling efficiency (>90%), radiochemical purity (>99%), and stability (>98%) for both diagnostic and therapeutic radioconjugates support rapid clinical translation.
