• Psychotic vulnerability identifies a clinically meaningful subgroup in pediatric OCD. • Psy+ cases showed poorer functioning and worse premorbid adjustment. • Verbal learning and memory differences were most evident in the Psy+ subgroup. • Negative psychosis-spectrum symptoms were linked to poorer neurocognitive performance. • Neurocognitive variability was related more to psychotic vulnerability than OCD severity.
