• Structural racism and heteropatriarchy are interlocking systems of oppression. • Racialized heteropatriarchy (RHP) predicts cardiometabolic risk (CMR) for US women. • Racialized social status and sexual minority status modify the RHP-CMR relationship. • RHP predicts CMR for all Black women and for White sexual minority women. • There is minimal association between RHP and CMR among White heterosexual women.
