• Ventral and posterior thalamus mediate both sensory discrimination and hypersensitivity in pain. • Medial and intralaminar nuclei integrate sensory-affective signals and facilitate pain-related emotional comorbidities. • Emerging electromagnetic and acoustic tools advance thalamic-targeted neuromodulation for analgesia. • A precision neuromodulation framework is highly needed to advance chronic pain management.
