• Acute rejection (AR) after liver transplantation can affect the patient's quality of life. • Siglec-15 is downregulated in a rat model of acute rejection after liver transplantation. • Siglec-15 promotes the polarization of Kupffer cells (KCs) towards the M2 phenotype. • Siglec-15 promotes M2 polarization of KCs through the DAP12/IRAK-M axis. • Siglec-15 overexpression alleviates acute rejection and improves survival in liver-transplanted rats.
