• Elucidated the dual role of CO in tumor progression and revealed its tumor-promoting and antitumor mechanisms. • The application of CO nanogenerators in spatiotemporally targeted cancer therapy via endogenous and exogenous stimuli was summarized. • The synergy of CO nanogenerators with other cancer therapies was summarized, and future challenges and research prospects were proposed.
