• Small bowel involvement is highly prevalent in systemic sclerosis and is characterized by impaired motility, including reduced contractile amplitude, abnormal phase III activity of the migrating motor complex, and an inadequate postprandial response. • No correlation was observed between esophageal and small bowel dysfunction, supporting the concept that gastrointestinal dysmotility in systemic sclerosis is segmental. • Histopathological studies support a multifactorial pathogenesis, implicating enteric neuropathy, smooth muscle atrophy, and vascular changes as key mechanisms driving small bowel dysfunction in systemic sclerosis.
