The adaptive physical activity programme in stroke (TAPAS): protocol for a process evaluation in a sequential multiple assignment randomised trial

引言:参与身体活动(PA)是卒中二级预防的基石。鉴于卒中的异质性,建议采用适应个体运动能力和相关合并症水平的PA干预措施。移动健康(mHealth)已被确认为支持卒中后PA的潜在方法。为此,我们采用序贯多重分配随机试验设计,开发了一种适应性mHealth干预措施以改善卒中后PA——卒中适应性身体活动计划(TAPAS)(Clinicaltrials.Gov NCT05606770)。作为卒中康复文献中首个采用此设计的试验,为这类适应性干预措施开展过程评估提供了机会。本过程评估的目的是考察TAPAS在社区步态功能恢复的卒中患者中的实施过程、变化机制及环境影响因素。方法与结果:依据英国医学研究理事会(MRC)过程评估框架,将采用定性与定量方法考察(1)TAPAS的实施过程及其内容(依从性、适应性、剂量和覆盖范围);(2)变化机制(参与者对干预的反应;中介因素;意外途径和后果)及(3)干预环境的影响。定量数据将以描述性方式呈现,例如运动课程的依从性。定性数据将通过半结构化的一对一或焦点小组访谈,在TAPAS参与者和干预实施者中收集。转录后的访谈将采用反思性主题分析法进行分析,并形成主要主题和子主题。伦理与传播:本研究已获得卫生服务执行委员会中西部伦理委员会的伦理批准(REC Ref: 026/2022)(25/03/2024)。研究结果将提交发表,并在相关国内外学术会议上报告。
