• NAT10 is the principal writer of ac4C on rRNA, tRNA and likely mRNA. • NAT10/ac4C regulates osteogenesis, osteoclastogenesis and cartilage homeostasis. • NAT10 inhibition shows preclinical promise in osteoporosis and osteosarcoma. • mRNA ac4C abundance remains debated; orthogonal detection methods are needed. • Single-cell and spatial epitranscriptomics are key future directions for ac4C in bone.
