Coronary artery disease (CAD) remains a leading cause of mortality worldwide, and its prevalence markedly increases with age. Atherosclerosis, the underlying pathological hallmark of CAD, is characterized by plaque deposition within the coronary artery walls, leading to vessel narrowing or occlusion, ultimately resulting in myocardial ischemia or necrosis. Macrophage senescence, a key component of inflammaging, contributes to both the initiation and progression of CAD through multiple interacting mechanisms. This review systematically outlines the molecular and functional characteristics of macrophage senescence, focuses on its pathological mechanisms in CAD, and provides an in-depth exploration of therapeutic strategies targeting macrophage senescence for CAD management. It aims to offer a comprehensive overview of the relationship between macrophage senescence and CAD and provides emerging treatment perspectives.
