• PORT is associated with improved OS and CSS in pN1, cM0 HNSCC patients without adverse pathological features. • PORT is an independent prognostic factor for OS and CSS. • Patients younger than 60 years old, or with OPSCC/LSCC gain significant survival benefit from PORT in this cohort.
