To the Editor: Costalat et al. (May 9 issue)(1) report positive outcomes in the Large Stroke Therapy Evaluation (LASTE) trial, which investigated endovascular thrombectomy for unrestrictedly large strokes. Although this trial did not have an upper limit for infarct size as an inclusion criterion, the median infarct volume was 132 to 137 ml, which is less than half the size of the total middle cerebral artery (MCA) territory (approximately 300 ml in a normal adult(2)).
