• Ultrasound-targeted nanodroplet destruction (UTND) overcomes ICB resistance in MSS colorectal cancer. • UTND induces oxidative stress in tumor cells, activating the AGE-RAGE pathway in CAF_Pi16. • CAF_Pi16 secretes CCL7, which upregulates NKG2D on CD8+ T cells to enhance tumor killing. • This study reveals a novel UTND-triggered CAF-Pi16-CCL7-NKG2D axis with high translational potential.
