Shortly before immigrating to Canada, a previously healthy 3-year-old girl presented to a physician in a rural village in Punjab, India, with a nontender fluctuant mass on the right lower chest that enlarged over a 2-week period. There was no preceding trauma, fever, weight loss, or tuberculosis (TB) exposure. She had contact with domestic cattle and buffalo and drank buffalo milk that was always boiled. She was fully immunized per local guidelines. Empiric aspiration and antibiotics (4 days of intramuscular lincomycin followed by 8 days of amoxicillin-clavulanate) resulted in minimal improvement.
