• A four-pattern heart rate variability (HRV) classification was successfully replicated in a European sample of patients with somatic symptom disorder (SSD). • Patients with a low HRV pattern showed consistently higher somatic symptom severity, depressive symptoms, and SSD-related psychological distress. • HRV pattern groups differed in overall symptom burden but showed stable trajectories over 12 months without differential change over time. • Pattern-based HRV assessment may offer a feasible physiological marker to identify autonomic subgroups with elevated symptom burden in SSD.
