The profession of physical therapy has spent the better part of 2 decades fighting for direct access and debating the definition of primary care physical therapy. During this time, the American Association of Medical Colleges released a workforce projection estimating a physician shortage that could top 86,000, with more than 40,000 of those physicians identified as primary care providers.1 With appointment wait times on the rise and less than $0.05 of every health care dollar going to primary care, it is clear we are in the midst of a crisis. The 2025 Consensus Definition by O’Bright and colleagues highlights an opportunity and provides a partial solution to that problem, a working definition of primary care physical therapy (PCPT).2
