• Compared salivary proteomes of OLK patients and a well-defined cohort of OLK-associated OSCC patients. • Developed a non-invasive machine-learning salivary proteomic panel of five biomarkers to detect OLK malignant transformation. • This five-protein panel achieved an exceptional AUC of 0.89–0.94 in detecting malignant transformation. • scRNA-seq, 4NQO models, and an independent OSCC cohort validated the biomarkers and their oncogenic relevance.
