• PROSPECT-based intraoperative and post-cesarean analgesia protocols were implemented in a large academic center. • Intraoperative protocol adherence exceeded 90% and was associated with reduced postoperative pain. • Low postpartum adherence limited benefit, underscoring the need for multidisciplinary implementation and education.
