• This randomized controlled trial is one of the first to investigate the effectiveness of combining 810 nm low-level laser therapy with standard medication in patients with trigeminal neuralgia from a major tertiary care centre in India. • The addition of photobiomodulation resulted in greater reductions in both pain intensity and frequency of attacks compared to pharmacotherapy alone. • Patients receiving the combined treatment experienced faster pain relief and required lower doses of systemic medication. • These results suggest that 810 nm photobiomodulation could serve as a safe, non-invasive adjunctive therapy, offering improved outcomes in the management of trigeminal neuralgia.
