• The ABC score for pCCA consists of 1 point each for preoperative tumor size ≥25 mm, CA19-9 ≥500 U/mL, and WHO PS ≥1 • Higher ABC scores correlated with higher 90-day mortality and 6-month recurrence rates • Median OS for ABC-0 and ABC-3 was respectively 39 vs 13 months, and 5-year OS was 35% vs 11% • We should be reluctant to offer surgery to patients with an ABC score of 3
